Claim Procedure

Intimation

As soon as an accident takes place, the Broker/insurance company should be notified over phone/e mail. This should be followed by written notification either by letter or e mail.

In case of Motor Vehicle accidents, the driver or insured’s employee who is in the vehicle at the time of accident should be requested to note the details of location, cause of accident where possible. If a third party is involved, please do not admit liability but collect details of the third party and their insurers.

Please remove the vehicle to a safe place where it cannot be vandalized.

Thereafter 2 or 3 repair estimates are to be sent where the insured is not taking to their preferred Garage. Once the repair estimates are received, the Broker/insurance company will decide about the inspection either by their In-house officials or engage an independent Professional Loss Adjustor.

For non Motor claims, we will advise you the documents required. On receipt of important documents such as claim form and repair estimates, we will arrange either to depute a Surveyor to assess the loss or grant permission to proceed with repairs.

Important

To ensure a smooth and quick settlement of claim, remember to

  1. Please act as if you are not insured and take all steps to protect the property and minimize the loss.
  2. Please do not commence repairs without written authorization from the Insurers.
  3. Please do not dispose damaged parts/ salvage without written authorization from the Insurers.
  4. Please do not admit liability to third party without insurer’s concurrence.
  5. Please protect any rights of recourse/recovery that may exist against the third party as it can be subrogated to the insurers for recovery after liability is admitted by insurers.

Documents Required

The documents required to process various claims are listed below.

Motor

  1. Claim Form duly filled and signed
  2. Log book copy
  3. Drivers Driving Permit
  4. Drivers statement
  5. Police Report
  6. Police Sketch map
  7. IOV report
  8. Two/three quotations for repairs
  9. Original Tax Invoice/Receipt from repairer after the vehicle is repaired
  10. All correspondence from Third party (unanswered) where third party claim is involved.

P.S. Please do not admit any liability or make any offer or promise of payment without our prior written consent; and to forward all correspondence to us unacknowledged. (in case of third party involvement)

Workers' Compensation

  1. Pay slips for the month prior to and after the accident
  2. LD form 31,35,& 87 and LD form 78 for death claims only
  3. Claim form duly filled and signed
  4. Supervisors report
  5. Original medical receipts with Doctor’s report.
  6. Personal Accident
  7. Pay slips for the month prior to and after the accident
  8. Claim form duly filled and signed
  9. Supervisors report
  10. Original medical receipts with Doctor’s report.

All Risk

  • Claim form duly filled and signed
  • User's statement
  • Purchase Invoice when the item was originally purchased
  • Replacement Invoice
  • Police report in case of theft

Money

  1. Claim form duly filled and signed
  2. Manager's Report
  3. Police Report
  4. Internal Auditors Report
  5. Supporting Vouchers/Invoices
  6. Terminal benefits of suspected employees in case of infidelity.

Marine

  1. Claim Form duly filled
  2. Delivery Note

  6. Monetary claim lodged on the Carrier under registered post
  7. Printed copy of Bill of Lading (both sides)
  8. Packing list.
  9. Survey Report

Goods in Transit
